KidneyTel’s CKD program focuses on early identification of kidney disease and reducing the complications associated with this disease. Early detection, patient education and medical intervention may slow the rate of renal function deterioration in those patients in the early stages of kidney disease. A vital component of the CKD Program centers on patient education. Informed patients are healthier patients. Patients identified with CKD continue to work with their primary care physician and/or nephrologist for care and treatment. Additionally, a multidisciplinary team of professionals are accessed to support the patient’s care management. Patients work with our nurse care managers who address their unique needs and the needs of their caregivers. Individualized care plans are created for each patient based on the stage of CKD. These care plans are shared with the patient and physicians periodically. The nurse care managers provide extensive patient education through one-on-one consultations and the sharing of educational materials. This effective multidisciplinary and educational program better prepares patients for dialysis and increases their awareness about dialysis options and transplantation. The patient’s family and other caregivers are included in the program when possible so they are better able to assist the patient. By preparing the patient and their support team for ESRD, we aim to reduce the number of hospitalizations and emergency starts to dialysis. The ultimate goal is to enhance quality of life, improve health outcomes and save costs. |
