Embedded in the KidneyTel software are clinical care pathways that are associated with educational mailers specific to the pathway. These pathways may be triggered manually or automatically resulting in individual patient focused interventions and actions. The nurse care manager may choose from a set of twenty-five educational material subjects that include blood pressure, catheter reduction / access placement, diabetes, flu and pneumonia vaccination, nutritional status, transplant preparation, renal replacement options, smoking cessation, advance directives, fall prevention, depression, malfunctioning dialysis access, heart failure, infection, wounds and open sores, and post hospitalization follow-up.
