Message from our President & General Manager:

Over the last 8 years I have had the good fortune of working with a team of accomplished healthcare professionals and collaborating with some of the country's leading and most visionary Nephrology Practices, in the design and implementation of an innovative integrated care platform/program to improve the outcomes for people with kidney disease. The results and success of our recently concluded (five year) Demonstration Project conducted by the Centers for Medicare and Medicaid Services (CMS) has been a rewarding validation of our work, and our company's approach to integrated care. Our team is committed to the mission of "raising the standard of care for people with kidney disease" - to include the broader comorbidity management and psychosocial needs of our patients. And we have demonstrated that doing the right thing in this regard will also result in reducing the cost of care for payers.

We are encouraged that several concepts emerging from the recent Affordable Care Act (Patient Centered Health Homes, Accountable Care Organizations, Chronic Disease Pilots) are positioning the U.S. healthcare system to continue in the direction that our company has worked towards for quite some time. While we are pleased with our accomplishments these past five years, we believe there is much more work to do and great opportunity ahead on continuing to improve outcomes for people with kidney disease through effective integrated care management. We remain energized and committed to our mission. With the outstanding results from the demonstration, we look for ward to bringing these proven programs to all aspects of care for people with renal disease.

I would also like to add a word of appreciation to Bob Farrell who preceded me in leading this program. His vision and leadership were the key elements that helped enable our success.
