Renaissance Health Care, Inc. is a NCQA accredited disease management company providing specialized services to kidney patients. Chronic Kidney Disease is a public health problem affecting roughly 20 million American adults. The spectrum of CKD includes five stages of disease, beginning with individuals with mild kidney damage progressing through those with kidney failure, more commonly referred to as End-Stage Renal Disease.

Renaissance Health Care emerged in 1996 as an active partnership between some of the country's leading nephrologists and Fresenius Medical Care North America. In January of 2006 RHC merged with Optimal Renal Care to become the largest provider of Care Management services to patients with Chronic Kidney Disease. RHC became a wholly owned subsidiary of Fresenius at this time.
With our Fresenius affiliation, comprehensive performance results and unique pre-ESRD program, Renaissance Health Care provides unparalleled services in renal disease management. Renaissance Health Care is a national company with contracts reaching from coast to coast. RHC's corporate headquarters are located in Lexington, MA with the operational offices still located in Broomfield, Colorado, a suburb of Denver.
